For this assignment, you will develop a treatment plan for yourself this semester. This treatment plan will include at least two goals, SMART objectives, and evaluation strategies. In module 6, you will provide the instructor with part I of this assignment, a copy of your treatment plan, and a pre-evaluation tool utilized to determine your current level of functioning.
Part 1 will include:
1. A treatment plan for yourself using one of the two treatment plans samples provided in class (module 4). For this treatment plan, you must include two goals, identify objectives for each goal utilizing the SMART framework, and provide evaluation strategies.

2. In addition, you will identify an evaluation tool (standardized measurement tool) that will help you identify baseline data for one of your goals. For example, if your goal is to reduce your test-taking anxiety, you might complete the Westside Test Anxiety Scale prior to the start of your interventions (pre-test). You will submit your completed survey with your treatment plan.
