SCENARIO

My scenario is the collection of patient data before admission. It’s often very important to have pre-details information of patient’s medical history, medication, disease, general health condition, social background, and so on before admission (McGonigle and Mastrian, 2017). Data may come from primary or secondary sources; from the patient directly or from family members, or from a nurse team who have worked with the patient in the past. Friends may also play a role in data collection who may or may not be in direct relation with family members. Data may also be collected from the paper charting system, diagnoses, lab results, and electronic medical records may populate data in and assist in the assessment of patient medical history. With all of the information acquired either from the primary or secondary data, one can then analyze what has been done in the past to develop a plan of care for the patient. Knowing what has worked or failed is crucial to developing a successful plan and achieving a better level of care for the patient.

Clinical reasoning and judgment are the thinking processes and strategies a nurse leader use to understand data and choose between alternatives with regard to identifying patient problems in preparation for making nursing diagnosis and selection of nursing outcomes and interventions (Benner, Hughes & Stuphen, 2007). Beyond observation, clinical judgment allows the nurse to associate pieces of information to review them, establish relationships with known facts, and analyze and interpret the data at hand from both a critical a rational perspective. McGonigle and Mastrian (2017) shared that the steps to using information, applying knowledge to a problem, and acting with wisdom form the basis of nursing practice science. Knowing where patient health condition stands is key to developing a starting point and creating a care plan that the patient and care team feel comfortable with.
