The primary focus of a community health nurse is to improve the health of the community. To do this involves using demographic and epidemiological findings to assess the community’s health and diagnose its needs. This assessment is aligned with the module objective “Discuss the elements of practice in community/public health nursing.”
This assignment is not a paper, so there is no cover page. All of the work is completed by completing the attached template. As part of the assignment, you will complete the following tasks: typing data, an analysis of the findings with four (4) references, and community health nursing diagnoses directly on the template.
Compile a range of relevant demographic and epidemiological data found on the community assessment rubric for this assignment. Use the websites listed below, as well as other websites (you can also do a Google search to find the health data you are looking for), gather information about your county and compare it with your state and national numbers.

Complete the assignment using the attached template on STEP 2.
Once you have obtained the required data, in the Analysis Section of the template, write a summary of findings in the space provided. Click on the analysis space and begin typing your summary. Identify actual and at risk health problems from the data that are relevant to your community.
Below the analysis summary formulate 2 (two) community health diagnoses using the data you have collected. Use the Figure 6-3 format in Chapter 6, pp.103 of your textbook.

You should be able to locate most of the requested information using the listed websites or by using a Google search. Include the website address, URL link or the link that appears in your server in the table. It is best to search city-data sites first for your city information.
You must include the website for each area of the table or at least put the same as above if utilizing the same resource. Blank resource areas will be given a point deduction.
For the first section (which is sometimes the hardest to locate, so you may want to start on other sections first), report your findings for hospitals and physicians in the statistical format found when you research it, and do not worry about the number of people per 1,000 if you are unable to find that statistic. After you obtain your data, complete a 600-word analysis identifying the key risks and concerns supported by your data. You need to describe more than one risk, please. You will need to use 4 references with citations in this area, as both are part of the rubric. These need to be in APA format. This area can be typed directly into the grey box on the template. It will expand as you type with the citations embedded in the body of the work and the references listed at the end.
The analysis should be a summary of the key risks and concerns that you can support based upon the statistical data you have retrieved. Please make sure that you cover more than one risk or concern in this area.
