An explanation of your agency and the services offered

A description of your client to include demographics, presenting problem, goal, legal/ethical considerations, assessment, and proposed treatment/social services delivery plan and the inclusion of termination plans if applicable

An explanation of whether interacting with your client demonstrated social work practice skills

Identification of potential social work skills not demonstrated in your agency or field placement to include a proposed professional development plan

Description of your client: 18 African American female

Demographics: single with 1 female child 7 months old

Presenting problem: domestic violence, child not meeting milestones, and mother with child lives with boyfriend

Goal: counseling for domestic violence, Safe House, therapy for the child, education information for college

Legal/ethical: contact DCS due to the violence with a small child in the home, and Legal Aide for restraining order

Few of these teen moms are left with minimal support from family, friends, the child’s father, and friends.
