Salley is 28 years old and is currently working with Dr. Jones, a psychiatrist for
depression. Salley believes much of her depression is caused by being
overweight. She would love to lose about 20 pounds, feeling this would help to
resolve her low self-esteem and subsequent depression. Dr. Jones disagrees,
believing at most Salley might benefit from a five-pound weight loss. It is
because of Salley’s obsession with her weight, Dr. Jones has been reluctant to
prescribe an antidepression medication for her (as a known side effect with this
type of medication is weight gain.) Dr. Jones has been using Cognitive
Behavioral Therapy (CBT) to treat Salley. In today’s session, Salley demands Dr.
Jones prescribe her Ozempic, which is a drug used in the treatment of type II
diabetes. A known side effect of Ozempic is weight loss, and for this reason, the
medication is becoming increasingly difficult to obtain and some diabetics can’t
get access to it.     
Keeping in mind the discussion from Chapter 5: “The Dual Continua Model” by
Keyes, who gets to determine the goals of mental health care, the provider, the
patient, the insurance provider? If you were Dr. Jones, how would you respond
to Salley?
