Since publication in 1998 of the seminal report from the Institute of Medicine (IOM), “To Err Is Human: Building a Safer Health System by L. T. Kohn, J. M. Corrigan, and M. S. Donaldson on the issue of medical errors and adverse events, much focus and effort by government and private institutions has been to improve patient safety and quality of care.

Since publication of “To Err is Human”, several other important reports were published:

The report, “Free From harm” which was published by the National Patient Safety Foundation goes on to provide information on patient safety. The journal goes on to state that patient safety is one of the most pressing issues which is plaguing the health care system.
