Cost, Access, and Quality

INTRODUCTION

Carefully read the Structure and Funding of Hospitalist Programs, Case # 20 located on page 293 in Darr, Farnsworth, & Myrtle’s Case book. This case involves organizational strategy in the context of hospital management. It provides an opportunity to investigate and understand the organization of inpatient healthcare delivery in the United Sates, not only regarding hospitalists but also looking at wide-ranging issues, such as interorganizational relationships between units at an academic medical center, patient flow through emergency departments, alternative funding sources and revenue generation, and work-hour restrictions on medical residents. An academic medical center must decide how to structure and fund hospitalist services in the context of its relationship with an affiliated school of medicine; the history and content of hospitalist functions; and other revenue that might be derived from hospitalist services, even while considering several alternate strategies.

UNIT LEARNING OUTCOMES

ULO #3: Pint out the relationship between access, cost, and quality in health care. (CLO 7)
ULO #4: Consider the dimensions of quality and cost-effectiveness in formulating recommendations for strengthening process outcomes (CLO 7)

DIRECTIONS
You are a consultant hired to assess the hospitalist program and make recommendations regarding its organization and future, including the level of need and justifiable subsidies.
Prepare a report with your recommendations considering the following questions:
Identify key stakeholders. What value or benefits do stakeholders derive? Show the relationship among Dogwood, the School of Medicine, and various stakeholders in tabular or graphical format.
Can hospitalist programs at Dogwood be an integral part of cost-effective and efficient inpatient care? Under what circumstances?
What information is missing or may be needed for the consultant’s initial report?
Use the Kaplan and Norton (1996) “Balanced Scorecard” framework and identify specific dimensions and metrics you could use to quantify the impact and performance of the hospitalist program.
Identify advantages and disadvantages of expanding the hospital medicine program to include a procedure service. Should co-management services with neurosurgery be considered? What questions must be asked? Explore the School of Medicine and Dogwood perspectives.
Identify strategic recommendations for the hospitalist program and suggestions for implementation at Dogwood. Develop recommendations for Dogwood regarding financial arrangements and future development of the program.
