Question 1
There are several issues facing the HCOs among them being Population Health Management. It is defined as a set of mechanism and process that are set to improve the health of a distinct population in a given area. Population health management is a must-have competency for HCOs of all sizes, specialties, and types. Developing and implementing a defined Population Health Management program is not easy especially for HCOs operating on limited staff and a clutched budget. On this aspect, efficiently managing patients suffering from complex chronic diseases, a tendency of patients ending up in hospitals over and again, and limited access to services requires healthcare providers to adjust fee-for-service workflows significantly; on the other hand, they should rely on collaborative and other team-based skills.
